29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) At Lot 18 Lafayette Rd, Destin, Florida: On or about January 1, 2022 and at times prior; the employer allowed his employees to work under others without hard hats while conducting framing activities exposing his employees to struck-by hazards from falling tools or materials.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) At Lot 18 Lafayette Rd, Destin, Florida: On or about January 1, 2022 and at times prior; the employer allowed his employees to perform framing activities without fall protection exposing his employees to a fall hazard of 13 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) At Lot 18 Lafayette Rd, Destin, Florida: On or about January 1, 2022 and at times prior; the employer allowed his employees to use extension ladders to gain access to the roof and the rails of the ladders did not extend at least three feet above the roof landing to provide safe access exposing his employees to a fall hazard of up to 13 feet to the ground below.
