Worksite located at 123 South 20th St. Irvington, NJ 07111, Dust Room- Vertical Baler Press Machine  a) On or about 12/3/2021 an employee standing on top of  the Vertical Baler Press Machine 6-2068 while repositioning carboard to be compressed, fell approximately 6 feet into the machine.  The employee was not provided with fall protection during the operation.
