29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a) 45 Main St., Milford, MA: On or about January 3, 2022, an employee was observed using a fiberglass extension ladder, to enter a window ,to access the building and the ladder did not extend 3 feet above the upper landing surface. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d) (1).

29 CFR 1926.1053(b)(5)(i):Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): a) 45 Main St., Milford, MA: On or about January 3, 2022, the employer had not ensured that employee(s) using portable extension ladders to access an upper level and were installed in manner in which the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder. An employee was observed using a fiberglass extension ladder, which was placed at a 2 to 1 angle (15'10" up and 8' out) to access an upper landing surface. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d) (1).
