29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) 30 Forbes Rd., Northborough, MA: On or about January 3, 2022, 2 employees were observed loading a makeshift dumpster with roofing debris at the roof edge without any fall protection in use, more than 24 feet above the ground below.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d) (1).

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record:  a) 30 Forbes Rd., Northborough, MA: On or about January 3, 2022, 3 employees were observed roofing this residence, and the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for any of the exposed employees.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d) (1).
