29 CFR  1910.28(b)(3)(ii): The employer failed to protect each employee from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems;  On or about January 5th, 2022 the employer of Sajacks Service LLC at 607 Shue Drive in Anna, Ohio failed to maintain fall protection for the service pits by having an adequate cover or guardrail system to prevent employees working in the service bay area from falling into the pits causing injury including but not limited to fractured bones.

29 CFR  1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields, were not maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about January 5th, 2022 the employer of Sajacks Service LLC at 607 Shue Drive in Anna, Ohio failed to maintain adequate personal protective equipment for employees handling and moving chemical products inside of the pump room. This includes maintaining PPE like gloves and faceshields free from dirt and dust and providing employees protective clothing like boots or aprons made of impervious materials for chemicals including but not limited to the toxic and corrosive product Aluminum Brightener, which contains sulfuric and hydrofluoric acid.

29 CFR  1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  On or about January 5th, 2022 the employer of Sajacks Service LLC at 607 Shue Drive in Anna Ohio failed to maintain sufficient clearance for the electrical breaker box and the space in front of the breaker box placing a workbench in front of it delaying employees to access the circuit breakers inside the box in case of an emergency.
