The employer failed to ensure the medical evaluation obtained all the information requested required by Sections 1 and 2, Part A of Appendix C including but not limited to current or historical pulmonary or lung problems and current or historical cardiovascular or heart symptoms. The employer provided and required healthcare professionals, including but not limited to Registered Nurses to wear N-95 respirators while providing direct care to suspected and confirmed positive COVID-19 residents.  The violation occurred on 12/7/2021 and thereafter.
