29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  Galloway Brothers Inc. located at 2717 Railroad Road in Sandusky, Ohio:  Instance A  On or about January 6, 2022 the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees handling lead. Employees tasked with removing lead weight from the rims of wheels were exposed to hazards associated with lead such as, but not limited to headaches, fatigue, abdominal pain, and possible lead poisoning.  Instance B  On or about January 6, 2022 the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees engaging in torch cutting and grinding operations. Employees tasked with torch cutting and grinding of metal material in were exposed to hazards associated with lead such as, but not limited to headaches, fatigue, abdominal pain, and possible lead poisoning.

29 CFR  1910.243(c)(3):Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:  a) Galloway Brothers Inc. located at 2717 Railroad Road in Sandusky, Ohio:  On or about January 6, 2022 the employer did not ensure that guards for right angle portable grinders were located between the wheel and the operator. Employees were exposed to struck by hazards when operating Metabo 15-150 Quick angle portable grinders without guarding in the event where the abrasive wheel would break.
