29 CFR 1910.132(d)(2):The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: Galloway Brothers Inc. located at 2717 Railroad Road in Sandusky, Ohio: Instance A On or about January 6, 2022 the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees handling lead. Employees tasked with removing lead weight from the rims of wheels were exposed to hazards associated with lead such as, but not limited to headaches, fatigue, abdominal pain, and possible lead poisoning. Instance B On or about January 6, 2022 the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees engaging in torch cutting and grinding operations. Employees tasked with torch cutting and grinding of metal material in were exposed to hazards associated with lead such as, but not limited to headaches, fatigue, abdominal pain, and possible lead poisoning.
