29 CFR 1910.132(d)(2):The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done. (a) Warehouse - On or about January 5, 2022 and times prior, the employer did not verify that the required workplace hazard assessment was performed through a written certification. Employees were required to move heavy bulky material throughout the warehouse and the employer failed to asses the requirement for safety shoes, thereby exposing employees to struck-by and crushing hazards.
