29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition.   a) Warehouse - On or about January 5, 2022, the warehouse floor was not maintained in a safe manner as there were large cracks throughout several aisles in the facility.  Employees were thereby exposed to hazards associated with trips, slips and falls, and the loss of control of material handling devices, including forklifts.
