OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns from accidental contact with hot steam pipes:  On or about February 24, 2022, at an establishment located at 610 Railroad Road in Mattoon, Wisconsin; the employer did not ensure boiler operator employees were protected from severe burn hazards due to potential contact with a non-insulated or unguarded metal steam pipe. The metal steam pipe temperature measured up to 209.3 degrees Fahrenheit and ran horizontally approximately 4 feet above the walking surface alongside the Erie City boiler. The pipes were located in an employee walkway used to access the valves on the boiler.

29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a) At a worksite located at 610 Railroad Street in Mattoon, Wisconsin; the employer did not provide adequate protective equipment to ensure boiler operators were protected against potential thermal burns to their arms and faces while performing the blow down on the Erie City wood boiler.  b) At a worksite located at 610 Railroad Street in Mattoon, Wisconsin; the employer did not provide adequate protective equipment to ensure boiler operators were protected from chemical burns to the skin and eyes while changing out the boiler water treatment (FREMONT BWT 4391), return line treatment (FREMONT RLT 4793), and the boiler dispersant (FREMONT 9010).

Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.  a) At a worksite located at 610 Railroad Street in Mattoon, Wisconsin; the employer did not ensure employees were provided and required to use chemical resistant gloves when changing out the boiler water treatment (FREMONT BWT 4391), return line treatment (FREMONT RLT 4793), and the boiler dispersant (FREMONT 9010).

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.   At a worksite located at 610 Railroad Street in Mattoon, Wisconsin; in the sawmill, the employer did not ensure that a portable Dewalt Circular Saw (DW364) was inspected prior to being used on each shift for external defects. The outer jacket of the flexible cord on the Dewalt Circular Saw was damaged, exposing the employee using it to electrical shock hazards.
