29 CFR 1910.147(c)(7)(iii)(A)  Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures.   (a) On or about January 3, 2022, at the worksite located at 9009 N 51st Suite A, Brown Deer, WI, the employer did not ensure employees implemented energy control procedures prior to changing salt packaging material on the #2 Barrington Packaging Systems machine, thereby exposing workers to struck-by hazards.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from the unexpected energization, startup, or release of stored energy during machine servicing and maintenance activities.
