29 CFR 1910.147(c)(7)(iii)(A) Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures. (a) On or about January 3, 2022, at the worksite located at 9009 N 51st Suite A, Brown Deer, WI, the employer did not ensure employees implemented energy control procedures prior to changing salt packaging material on the #2 Barrington Packaging Systems machine, thereby exposing workers to struck-by hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from the unexpected energization, startup, or release of stored energy during machine servicing and maintenance activities.

29 CFR 1910.147(c)(7)(iv): The employer had not certified that employee training in the lockout/tagout procedures had been accomplished and kept up to date and shall contain each employee's name and dates of training: (a) On or about January 3, 2022, at the worksite located at 9009 N 51st Suite A, Brown Deer, WI, the employer did not ensure that hazardous energy control training was documented and certified, and also included the names of employees and the dates of training.
