On or about January 7, 2022, at the IT Room, the electrical service panel identified as UPS Breaker BOX - UP2 was not readily accessible. The electrical service panel was blocked with a safe box/file cabinet placed in the floor below and in front, obstructing access. On or about January 7, 2022, at the north hallway, the electrical service panel identified as Panel # 3 (240 VOLTS) had one, uncovered, unused circuit breaker opening, exposing the energizing bus bar. Employees were exposed to electrical hazards from an open electrical service panel with exposed live parts through unused openings.
