1910.179(j)(1)(i) Prior to initial use, all new and altered cranes were not inspected to insure compliance with the provisions of 29 CFR 1910.179(j). At Maintenance area, employees engaged in maintenance activities were exposed to struck-by and crushing hazards from a Engineered Material Handling 10 ton crane, which did not have either frequent or periodic inspections.

29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: At the Maintenance Shop, employees engaged in grinding activities at the Dewalt Pedestal grinder were exposed to struck-by and caught in hazards in that the tongue guard was adjusted to approximately 3/4 of an inch from the grinding wheel.
