OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to slip/trip/fall hazards and/or being struck-by hazards:  a)  Warehouse 2A and 2B;  The employer exposed employees to struck-by hazards in that adequate lighting was not  provided when employees were assisting in the loading of forklifts, performing inventory, and moving vehicles in and out of the warehouse.  b)  Warehouse 2A and 2B;   The employer exposed employees to slip, trip and fall hazards in that employees in the warehouse were working in areas when wet  surfaces were present and adequate lighting was not provided.

29 CFR  1910.22(d)(2):29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:  a) On or about  March 3, 2022 and times prior; Warehouse #2 inside storage area: The employer failed to ensure numerous damaged concrete floor areas involving holes and ruts/uneven areas throughout both sides and the center area where employees walk were repaired to prevent potential fall hazards to the same level.  b) On or about  March 3, 2022 and times prior; Warehouse #2 outside area: The employer failed to ensure numerous surface areas of damaged concrete creating holes where employees walk, load, and unload material were repaired to prevent potential fall hazards to the same level.  c) On or about  March 3, 2022 and times prior; Warehouse #2B outside area: The employer failed to ensure a large surface area of damaged concrete creating holes/uneven areas where employees walk was repaired to prevent potential fall hazards to the same level.
