29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) D&D Commodities Limited at 420 O Street, Greeley, CO 80631: On January 6, 2022, the employer did not ensure that machine specific, step-by-step procedures for the Hamer 2080 Bagging Machine were utilized for the control of potentially hazardous energy when employees cleared poly jams. An employee sustained an amputation injury while clearing out poly from the machine. This condition exposed the employee to an amputation hazard. (b) D&D Commodities Limited at 420 O Street, Greeley, CO 80631: On or before January 7, 2022, the employer did not ensure that machine specific, step-by-step procedures for the Hamer 2080 Bagging Machine were utilized for the control of potentially hazardous energy when employees cleared poly jams. Procedures were not utilized on a routine basis when performing this task. This condition exposed employees to amputation and other serious hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) D&D Commodities Limited at 420 O Street, Greeley, CO 80631: On or before January 7, 2022, the employer did not conduct a periodic inspection at least annually of the Lockout/Tagout (LOTO) procedures used by maintenance personnel to ensure that the procedure was being implemented while clearing poly jams from the Hamer 2080 Bagging Machine. The procedure was commonly not utilized when performing this task. On January 6, 2022, an employee sustained an amputation injury while clearing poly from the machine. This condition exposed employees to amputation and other serious hazards.
