29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about March 29, 2022, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, employees were exposed to struck by, caught in or between, and crushing injuries from pneumatic and mechanical energy sources when performing maintenance/service activities and mold and die changeovers on the Van Dorn (Model 300-RS - Serial # 300-RS-25-2126) press machine #203 injection molding machine. b) On or about March 29, 2022, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, employees were exposed to struck by, caught in or between and electrical shock hazard from pneumatic, electrical and mechanical energy sources when performing maintenance/service activities on the Novatec (Model HB25 - Serial #3A3899-4941) Dryer #0028 (208 V) within the manufacturing area. c) On or about March 29, 2022, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, employees were exposed to struck by, caught in or between, and crushing injuries from pneumatic and mechanical energy sources when performing maintenance/service activities and mold and die changeovers on the Van Dorn (Model 250-RS - Serial #250-RS-20F-VB-368) press machine #201 injection molding machine.

North Ridgeville OH 44039, employees were exposed to fire, respiratory and burn hazards in that the employer did not ensure employees were provided with training on the general principals of fire extinguisher use and the hazards involved with incipient stage firefighting on an initial employment or annual basis. On or about January 28, 2022, employees were exposed to an incipient stage fire on Van Dorn (Model 300-RS - Serial # 300-RS-25-2126) press machine #203, requiring the use of portable fire extinguishers to extinguish the fire.

North Ridgeville OH 44039, employees were exposed to potential electrical shock hazards in that the employer was using separate temporary wiring/flexible cords in lieu of fixed wiring to power the Van Dorn (Model 300-RS - Serial # 300-RS-25-2126) press machine #203 injection molding machine loader system. b) On or about March 29, 2022, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, employees were exposed to potential electrical shock hazards in that the employer was using separate temporary wiring/flexible cords in lieu of fixed wiring to power the Novatec (Model HB25 - Serial #3A3899-4941) Dryer #0028. c) On or about March 29, 2022, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, the employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard is present, specifically, the categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Employees were exposed to forklift propane fuel cylinders, mineral spirits utilized for cleaning parts and tools, a mold cleaner product containing tetrachlorethylene, toluene, and acetone utilized for the lubrication of the mold casting and die machines, and Mold Saver containing xylene utilized for the lubrication of the mold casting and die machines.
