29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about October 3, 2023, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, employees were exposed to potential electrical shock hazards in that the employer was using temporary wiring/flexible cords in lieu of fixed wiring to power the IMS (Model: TC-0255 S/N: S35280000) plastics coloring tumbler machine. One temporary wiring/flexible cord plugged into a 240V outlet, was used to power the plastics coloring tumbler machine.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about October 2, 2023, employees at Dreco Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, did not ensure periodic audits of the energy control procedures were completed at least annually. Employees are required to perform mold change activities on machines including but not limited to, the , #203 Van Dorn plastic injection molding machine (Model 300-RS - Serial # 300-RS-25-2126), which require energy control procedures that have not been reviewed for deficiencies.  Dreco Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.147(c)(1), which was contained in OSHA inspection number 1571772, citation number one, item number one and was affirmed as a final order on May 23, 2022, with respect to a workplace located at 7887 Root Rd. North Ridgeville OH 44039.

29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about October 2, 2023, employees at Dreco Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, were exposed to crushing and struck-by injuries in that the employer did not ensure employees who service and maintain plastic injection molding machines such as, but not limited to, #203 Van Dorn plastic injection molding machine (Model 300-RS - Serial # 300-RS-25-2126) were trained in the skills required for the safe application, usage and removal of energy controls.  Dreco Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.147(c)(1), which was contained in OSHA inspection number 1571772, citation number one, item number one and was affirmed as a final order on May 23, 2022, with respect to a workplace located at 7887 Root Rd. 29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about October 2, 2023, employees at Dreco Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, were exposed to crushing and struck-by injuries in that the #203 Van Dorn plastic injection molding machine (Model 300-RS - Serial # 300-RS-25-2126) was not deenergized prior to servicing tasks such as, but not limited to, mold and die changeovers.  The employer did not ensure that energy control application steps were implemented to control hazardous electrical, hydraulic, pneumatic, energy sources of the machine's mold, die, platen and associated moving parts, prior to the servicing task - the employer did not: ?	Ensure employees had the method or means to control hazardous energy. ?	Physically operate energy isolating devices and affix lockout device(s) to control the energy source(s).  ?	Verify that isolation and de-energization of the machine or equipment had been accomplished.  Dreco Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.147(c)(1), which was contained in OSHA inspection number 1571772, citation number one, item number one and was affirmed as a final order on May 23, 2022, with respect to a workplace located at 7887 Root Rd. North Ridgeville OH 44039.

29 CFR  1910.1200(g)(2): The chemical manufacturer or importer preparing the safety data sheet did not ensure that it included the required headings and information in 29 CFR 1910.1200(g)(2)(i) through 29 CFR 1910.1200(g)(2)(xii)  On or about October 3, 2023, at Dreco, Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, employees were exposed to potential chemical hazards in that the employer did not maintain a safety data sheet for Stoner Dry Mist Silicone Release (E202) and Stoner Release & Paint (E313). The employer did not maintain safety data sheets that meet the requirements and information required in 1910.1200(g)(2)(i) through 1910.1200(g)(2)(xvi).
