29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift: On or about January 10, 2022, above the entrance of a commercial building, an employee was installing a sign, using an aerial lift without attaching to the bucket or boom using a lanyard. The employee was exposed to a fall hazard greater than 6 feet.
