29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.  Location - 4 Fletcher Street, Maynard, MA.  On or about January 10, 2022, employees were not provided safe means of access to the ladder jack scaffold.

29 CFR  1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.  Location - 4 Fletcher Street, Maynard, MA.  On or about January 10, 2022, the employer allowed employees to work on ladder jack scaffolding without adequate training by competent person to recognize hazards.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location - 4 Fletcher Street, Maynard, MA.  On or about January 10, 2022, five employees were exposed to a fall hazard of over sixteen feet,  from a residential roof where fall protection was not used.

29 CFR  1926.502(d)(15): At this job site, the anchorage used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5000 pounds per employee.  One employee was anchored to a vent pipe in lieu of being attached to a adequate support.   Location - 4 Fletcher Street, Maynard, MA.  On or about January 10, 2022, five employees used a single roof anchor point to attach their personal lifelines to that was not capable of supporting more than five thousand pounds and not independent of each other.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  Location - 4 Fletcher Street, Maynard, MA.  On or about January 10, 2022, the employer failed to ensure that five employees exposed to fall hazards were properly trained in the means and methods of fall protection equipment.
