29 CFR 1910.178(n)(1): The employer did not ensure all traffic regulations were observed and that a safe distance was maintained by employee(s) approximately three truck lengths from the truck ahead, and the truck was not kept under control at all times: a) Production Floor: On or about 1/10/2022, the employer did not ensure that the powered industrial truck operator was maintaining control of their truck when backing up and a safe distance, of approximately three truck lengths, was not maintained from the pallet jack ahead when they were working in a high pedestrian traffic area, causing an employee to sustain a fractured foot and leg when struck by the powered industrial truck.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of, the path of travel: a) Production Floor: On or about 1/10/2022, prior to backing up the Nissan powered industrial truck, the operator did not have a clear view of the path of travel, resulting in an accident leading to injuries to another employee, including a fractured foot and leg.
