29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) At the jobsite, employees were working in the bottom of the excavation installing cement retaining blocks approximately 10 feet deep without a protective system in place, exposing the employees to the hazard of a cave-in.
