Chicopee, MA. 01013: On or about January 12, 2022, and times prior, the employer did not ensure the Toyota and Nissan forklifts were removed from service due to needed repairs. a) The Toyota forklift had broken taillight lenses. b) The Nissan forklift tires are in disrepair and right rear tail light lenses are broken.
