This exposure was observed over a 296 minute sampling period. Zero exposure was assumed for the 184 minutes not sampled. The employer did not develop a noise monitoring program for those employees exposed over the action level of 85 dBA, on or about February 17, 2022. The employer did not ensure that each employee had access to labels on containers of formaldehyde and to safety data sheets, and was trained in accordance with the requirements of HCS and paragraph (n) of this section: a) Finishing Department: Employees were exposed to formaldehyde while performing cabinetry priming, painting, varnishing, and glazing operations, and the employer did not ensure that these employees were provided with hazard communication training specific to formaldehyde, on or about January 12, 2022. ABATEMENT CERTIFICATION REQUIRED ABATEMENT NOTE: Hazard communication training for formaldehyde shall address at least the following hazards: cancer; skin and respiratory sensitization; eye, skin and respiratory tract irritation; acute toxicity effects; and flammability.

ABATEMENT CERTIFICATION REQUIRED ABATEMENT NOTE: Formaldehyde training shall include all elements listed in paragraph (n)(3) of this standard and be repeated annually.
