29 CFR  1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  a) 687 E 9th St, Center, CO 81125: On December 17, 2021, and at times prior, employees were exposed to caught-in hazards from working directly next to an unguarded chain and sprocket while sorting potatoes. An employee suffered a hand injury when her hand was caught-in the chain and sprocket of the Spudnik Potato Conveyor, serial number 1250-55-1841.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) 687 E 9th St, Center, CO 81125: On December 17, 2021, Leroy Padilla, Foreman of Canon Potato LLC, failed to report the in-patient hospitalization of an employee within 24-hours a work-related incident. An employee had their hand crushed by an unguarded chain and sprocket and was taken to a hospital in Alamosa, CO, on December 17, 2021.
