29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a- 16 Autumn Park Boulevard, Building 4, Medford NJ:   Workers were exposed to struck by hazards when using pneumatic nailers, portable circular saws, etc. without eye or face protection. Condition observed on or about 01/12/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:   a- 16 Autumn Park Boulevard, Building 4, Medford NJ:   A worker was exposed to fall hazards, up to and in excess of 21 feet  7 inches to the ground surface below, when walking and working from a pump jack scaffold whose posts/legs were not set on an adequate foundation. Condition observed on or about 01/12/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Condition observed on or about 01/12/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a- 16 Autumn Park Boulevard, Building 4, Medford NJ:   Four workers were exposed to fall hazards, up to and in excess of 10 feet  9 inches to the ground surface below, when walking and working at and near the unprotected edges of a commercial structure's third floor balcony while wearing full body harnesses that were not attached to lanyards and an anchor.  Condition observed on or about 01/12/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.   Great Work Construction Corporation was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard  29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1342367, citation number 1, item number 5 and was affirmed as a final order on 01/09/19, with respect to a workplace located at 42 Lexington Court, Englewood NJ.
