29 CFR 1910.1200(b)(4)(iii):In work operations where employees only handle chemicals in sealed containers which are not opened under normal conditions of use, the employer did not ensure that employees were provided with information and training in accordance with 29 CFR 1910.1200(h) to the extent necessary to protect them in the event of a spill or leak of a hazardous chemical for a sealed container: a) 933 Broad Street, Portsmouth, VA 23707: On or around 1/13/21 and times prior to, employee(s) worked around, install, and cleaned up mercury containing Cool White T-12 34W fluorescent light bulbs without hazard communication training that covered the hazards of mercury exposure.

29 CFR 1910.305(a)(2)(ix):All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded. a) 933 Broad Street, Portsmouth, VA 23707: On or around 1/13/21 and times prior to, it was observed that the case lights throughout the facility are unguarded on their bottom edge. The unguarded nature of the lights contributed to an incident, two weeks prior to the inspection, which resulted in a light dislodging from its housing, falling, and breaking on an employee.
