On or about January 12, 2022, at jobsite located in 515 Rex Blvd., Elmhurst, IL 60126, the employer failed to certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards in approximately 20 feet while performing roofing activities on a new residential structure without using fall protection.
