29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.    On 13 Jan 2022, at a jobsite located at or near 2601 NW S Outer Rd Blue Springs, MO; an employee installing gutters along the edge of a roof was exposed to fall hazards. The employee was working inside of an aerial lift with a harness on but the lanyard was not attached to an anchor point of the aerial lift.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On 13 Jan 2022, at a jobsite located at or near 2601 NW S Outer Rd Blue Springs, MO; an employee accessing the roof to install sheet metal along the edge of a roof was exposed to fall hazards when the ladder he was using to access the roof was not extended at least 3 feet above the upper landing surface.

29 CFR  1926.501(b)(10): each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  On 13 Jan 2022, at a jobsite located at or near 2601 NW S Outer Rd Blue Springs, MO; an employee installing sheet metal along the edge of a roof was exposed to fall hazards.  The employee was working on the flat area of the roof, near the edge of the roof with a fall distance of greater than 6 feet, with no means of fall protection.     Precision Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 1461356, citation number 2, item number 1 and was affirmed as a final order on 09-11-2020, with respect to a workplace located at 600 W. 39th St., Kansas City, Missouri.  Precision Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1273329, citation number 1, item number 2 and was affirmed as a final order on 08-01-2018, with respect to a workplace located at located at 17700 E. Susquehanna Drive, Independence, Missouri.
