29 CFR 1910.29(e)(2): The employer did not ensure open hole(s) in a walking-working surfaces were covered securely to prevent accidental displacement: On or about January 7, 2022, AR-Letts Trucking, sub-contractor to Garcia Grain Trading, failed to ensure a floor hole in the pit was securely covered, as a result, an employee fell into the floor hole, sustaining multiple injuries.
