(a) Location - 300 Mishawum Rd, Woburn, MA 01801 On or about 01/06/2022, Employees were exposed to a fall hazard while working on a low-sloped roof without fall protection. Prime Roofing Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1394137, citation number 1, item number 1-a and was affirmed as a final order on 07/10/2019, with respect to a workplace located at 97 Pleasant Street, Concord, NH 03301.
