29 CFR 1904.29(b)(1):What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a) On or about November 29, 2021, the employer failed to provide a description for an injury that occurred on November 29, 2021, in that it failed to describe the object or substance that directly injured the employee, and the injury was placed on the 2022 OSHA 300 Log instead of the 2021 OSHA 300 Log. b) On or about December 24, 2021, the employer failed to describe the illness that made the employee ill in that COVID-19 was listed as a lung disease, and the illness was placed on the 2022 OSHA 300 Log instead of the 2021 Log.

29 CFR 1904.32(a): Basic requirement. At the end of each calendar year, you must: (1) Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified; (2) Create an annual summary of injuries and illnesses recorded on the OSHA 300 Log; (3) Certify the summary. a) On or about January 19, 2022, the entries for the 2021 OSHA 300 Log were not verified as complete and accurate in that two entries for November 29, 2021 and December 24, 2021 were not placed on the 2021 OSHA 300 Log but were placed on the 2022 OSHA 300 Log. Therefore, the two entries were not included in the 2021 OSHA 300A Summary. b) On or about January 19, 2022, the employer had not signed and certified the 2021 OSHA 300A Summary.
