29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a)	This violation occurred on or about January 13, 2022, at a jobsite located at 2937 South Griffins Lane, Heartland, TX; where employees doing roofing work on a two story residential home and using a Metabo nail gun were exposed to eye injuries by not wearing eye protection.  MARIO SOLIS VENTURA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.102(a)(1) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 14132893, CITATION NUMBER 1, ITEM 1A THAT WAS ISSUED ON JULY 1, 2019 WITH RESPECT TO A WORKPLACE LOCATED AT 706 FARSTEAD, ARLINGTON, TX, 76002.   THE FINAL ORDER DATE WAS JULY 30, 2019.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  This violation occurred on or about January 13, 2022, at a job site located at 2937 South Griffins Lane, Heartland, TX; where employees were using extension ladders that did not extend 3-feet above the landing.  Employees were exposed to a fall hazard: (a)	16-feet extension ladder located at the front of the house. (b)	24-feet extension ladder located at the side of the house.
