Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  (a) Location - 2 Curley Circle, Ayer, MA:  On or about 01/14/2022, employees were exposed to electrical shock hazards while using a flexible electrical cord that had the outer sheathing pulled away from the strain relief device, exposing the insulated conductors.

(a) Location - 2 Curley Circle, Ayer, MA:  On or about 01/14/2022, employees were not adequately protected against falls of greater than 6 feet above a lower level in the following instances:  a) Employees placing roof sheathing on a second story roof were unprotected against fall hazards.   b) Employees working on a second floor were exposed to fall hazards from open and unguarded window openings.   c) Employee working on the first floor were exposed to fall hazards from open and unguarded floor hole to the basement.

(a) Location - 2 Curley Circle, Ayer, MA:  On or about 01/14/2022, employees performing framing work were exposed to fall hazards of 10-feet when a portable ladder that had damaged side rails was not withdrawn from service.
