29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. a.) 900 Susquehanna Avenue, Huntingdon, PA 16652, on or about January 14, 2022, the employer did not maintain OSHA 300 injury and illness logs for CY 2021. b.) 900 Susquehanna Avenue, Huntingdon, PA 16652, on or about January 14, 2022, the employer did not maintain OSHA 301 injury and illness reports for CY 2021. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
