The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about January 11, 2022, the employer failed to guard the point of operation for tire "debeader" machines in the production building, in that:  a. the two hand controls were not functional for the east tire "debeader" machine in that the left control was disconnected and the  controls did not function as designed, to prevent the operator from entering the danger zone during the operating cycle, and   b. the two hand controls were not functional for the west tire "debeader" machine in that the right control was disconnected and the  controls did not function as designed, to prevent the operator from entering the danger zone during the operating cycle.  Due to these conditions,  machine operators can access the  danger zone during the operating cycle. On November 4, 2021, an employee suffered a finger laceration when his hand was pulled into the point of operation while reaching into the danger zone to unjam a tire that was "stuck" while operating the two hand control with the left hand and left forearm.
