29 CFR 1926.501(a)(2):The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. On or about December 23, 2021, the employer failed to ensure the cover of a hole adjacent to a permanent ladder in the area of the Skip Hoist Construction Project located at 8826 US-24 Mapleton, IL 61547, had the strength and structural integrity to support employees safely. The hole measured approximately 6.5 feet wide and 10 feet long, with a 24-foot, 6-inch drop to a lower level, and was covered with ?-inch thick plywood intended only as weather protection for the area below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about December 23, 2021, the employer failed to ensure all employees exposed to fall hazards while working at the Skip Hoist Construction Project located at 8826 US- 24 Mapleton, IL 61547, were trained to recognize the hazards of falling and the procedures to be followed to minimize those hazards.
