29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At 2225-2555 County Rd 1149 , Troy, Alabama:  On or about January 14, 2022,  the employer  did not ensure that hard hats were used by employees on the ground level while employees on the roof above were performing roofing operations and receiving roofing materials from the ground level; exposing ground employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At 2225-2555 County Rd 1149, Troy, Alabama: On or about January 14, 2022,  the employer allowed his employees to perform roofing activities without fall protection (i.e.: harness, lanyard with anchor);  exposing employees to fall hazards up to approximately twenty-three feet (23') to the ground below.

29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that is reasonably accessible in terms of time and distance to the worksite, which is available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, was not available at the worksite to render first aid:  a) At 2225-2555 County Rd 1149, Troy, Alabama:  On or about January 14, 2022, and times prior, the employer failed to ensure that there was an infirmary, clinic, or hospital in near proximity to the remote workplace or a person or persons adequately trained by documentary evidence, to render first aid for exposed employees to serious safety and health hazards at the remote workplace.
