29 CFR  1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a.)  Working on a pitched, newly constructed residential roof, on or about January 13, 2022 and at times prior, the employer did not conduct frequent and regular inspections of the job site, materials and equipment being used, thus exposing employees to a 10.2 foot fall hazard.

29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a.)  Working on a pitched, newly constructed residential roof, on or about January 13, 2022 and at times prior, employees were not adequately trained to recognize the hazards associated with not using a complete fall protection system, thereby exposing employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a.)  Working on a pitched, newly constructed residential roof, on or about January 13, 2022 and at times prior, the employer exposed employees to fall hazards, in that the employer failed to ensure the rails of an extension ladder extended three feet above the upper landing surface while employees used the ladder.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a.) Working on a pitched, newly constructed residential roof, on or about January 13, 2022 and at times prior, the employer exposed employees to fall hazards by not ensuring employees installing shingles at a height of 10.2 feet were utilizing fall protection.    Jordan Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1454638, citation number 1, item number 1and was affirmed as a final order on April 20, 2020, with respect to a workplace located at 104 Rolling Hills Drive Newnan, GA 30263 (Building #3).
