29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) On or about 1/14/2022 in Russellville, Arkansas at the jobsite located on B Street at 200 North Frankfort Avenue, employees of Blackstone Construction working in an excavation approximately six and one-half feet deep with vertical sides were using a damaged Werner 20-foot aluminum extension ladder for access and egress from the excavation, exposing them to the hazard cuts from the broken rung.
