29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) On January 12, 2022, the employer did not ensure that its sub-contractor protected its employees from cave-in hazards while working outside of trench boxes to install a new manhole and 8" gravity fed sewer line in an excavation approximately 20' deep with nearly vertical walls and Type C soil. Note - Abatement certification AND documentation are required for this item.
