1910.147(d)(4)(i) Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.  Swift Beef Company - 555 S. Stuhr Rd, GRAND ISLAND, NE  68801 - Knock Box Station Area - Employees are exposed to crushing injuries created by the hydraulic restrainer lid by not locking out the lid.  On or about January 14, 2022, an employee suffered fatal injuries from the restrainer lid while attempting to knock a stuck cow in the restrainer chute.
