29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On January 18, 2022, at a workplace located at 420 West Main Street Coleman, WI, in the Fabrication Department: employees walking over oil dry and oily spots on the floor due to oil leaking from the vertical saw to access the scrap shelving, exposed them to slip, trip, fall hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a) On January 18, 2022, at a workplace located at 420 West Main Street Coleman, WI, in the CNC Department: On the Doosan Puma 240 CNC Turning Center, an employee was exposed to rotating parts hazards from the unguarded rotating spindle. The interlock switch installed on the sliding operator access door had been disabled and permitted the door to be open while the spindle was in motion. b) On January 18, 2022, at a workplace located at 420 West Main Street Coleman, WI, in the CNC Department: On the Shenyang No. Machine Tool Works CNC machine, an employee was exposed to rotating parts hazards from the unguarded rotating spindle. The interlock switch installed on the sliding operator access door had been disabled and permitted the door to be open while the spindle was in motion.
