29 CFR 1910.502(q)(2)(ii)(A):The COVID-19 log did not contain, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced. a) Aveanna Healthcare, LLC Located at 4 Neshaminy Interplex Suite 204 in Trevose, PA: On or about January 12, 2022, the employer failed to ensure the COVID-19 log provided the location where the employee worked, and one form of contact information for employees that tested positive for COVID-19. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
