29 CFR  1910.219(c)(2)(i):  All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires:  (a) At a facility located at 16167 W. Rogers Drive, New Berlin, WI 53151; the employer did not enclose the sides, top and bottom of horizontal shafting less than 7 feet from the floor. In the mechanical area behind the Melanite line, the horizontal shaft providing power transmission from a motor to an exhaust system pulley was not protected or enclosed to prevent employees from contacting and becoming entangled in the rotating shaft.

29 CFR  1910.219(d)(1):  Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section (standard guards - general requirements):  (a) At a facility located at 16167 W. Rogers Drive, New Berlin, WI 53151; the employer did not guard pullies less than 7 feet from the floor. The rotating portions presented by exhaust system pullies, which were transferring power from a horizontal shaft, were not guarded to protect or prevent employees from contacting and becoming entangled in the rotating portions of the pullies.

29 CFR  1910.219(e)(1)(i):  Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section:  (a) At a facility located at 16167 W. Rogers Drive, New Berlin, WI 53151; the employer did not guard belts less than 7 feet from the floor. The moving portions and ingoing nip points presented by exhaust system belts, which were transferring power between pullies, were not fully enclosed to protect or prevent employees from contacting and becoming entangled in the moving portions of the belts or the ingoing nip points between the belts and pullies.

Rogers Drive, New Berlin, WI 53151; employee training did not include methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area. Employees exposed to carbon monoxide, as a byproduct of combustion, in the workplace were not trained how to detect the presence or release of the odorless and colorless hazardous chemical.

Employees exposed to carbon monoxide, as a byproduct of combustion in the workplace, were not trained on the health hazards of carbon monoxide such as but not limited to chemical asphyxia, headache, tachypnea, nausea, lassitude (weakness, exhaustion), dizziness, confusion, angina, or syncope.
