29 CFR  1910.178(m)(5)(ii): A powered industrial truck shall not be left unattended when the operator is 25 ft. or more away from the vehicle which remains in view, or whenever the operator leaves the vehicle and it is not in view.  Location 130 Washington Street, Ayer, MA.  On or about January 19, 2022, employer operating a powered industrial truck raised a load to the second floor and left the cab of the powered industrial truck a distance greater than twenty five feet.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location - 130 Washington Street, Ayer, MA.  On or about January 19, 2022.  Instance A: three employees were working/walking on a second floor deck at a height greater than ten feet above lower level with no fall protection.  Instance B: three employees were exposed to fall hazards greater than fourteen feet above lower level from unprotected windows.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them.  Location - 130 Washington Street, Ayer, MA.  On or about January 3, 2022, The employer did not provide adequate training for the employees exposed to fall hazards.
