29 CFR  1926.1053(b)(3):  Ladders were loaded beyond the maximum intended load for which they are built:  a) At this jobsite, three employees were exposed to fall hazards of approximately 4-feet to 21-feet when they used a Werner 24-foot aluminum extension ladder at the same time, exceeding the maximum capacity of the ladder.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)  At this jobsite, employees were exposed to fall hazards of approximately 11-feet to 21-feet and greater when they worked without using fall protection.   Luis Diaz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1503303, citation number 1, item number 1, and was affirmed as a final order on April 28, 2021, with respect to a workplace located at 5007 Mossy Bridge Dr., SPRING, TX 77379.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a) At this jobsite, employees were exposed to fall hazards of approximately 11-feet when they used an aluminum extension ladder at the front of the house, the rear of the house, and the west side of the house that were not extended three feet above the roof landing.   b) At this jobsite, employees were exposed to a fall hazard of approximately 21-feet when they used an aluminum extension ladder that was not extended three feet above the roof landing.   Luis Diaz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1503303, citation number 1, item number 2, and was affirmed as a final order April 28, 2021, with respect to a workplace located at 5007 Mossy Bridge Dr., SPRING, TX 77379.
