29 CFR  1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.  (a) Location - 63-65 Merriam Street, Somerville, MA:  On or about 1/19/2022, the employer allowed employees to work from a pump jack scaffold with 2x8 in planks containing knots and other imperfections. These scaffold planks were not graded by a recognized lumber grading association for this use and were not capable of supporting 4 times the maximum intended load.

29 CFR  1926.451(b)(2)(i): Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold was not at least 12 inches (30 cm) wide.  (a) Location - 63-65 Merriam Street, Somerville, MA:  On or about 1/19/2022, employees working from a pump jack scaffold were not protected from fall hazards where the scaffold platforms were less than 12 inches wide. The employer was using 2x8 nominal wood planks for scaffold platforms.
