29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  a) Jobsite located at 1719 Seybert Street, Philadelphia, PA.- The employer did not maintain an accident prevention program to help protect their employees from hazardous working conditions. Employees were exposed to soil collapse hazards while installing a new sewer service in a trench that was approximately 7 feet deep that was not protected from collapse.  Condition was observed on or about January 20, 2022.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jobsite located at 2601-2617 Poplar Street, Philadelphia, PA.- The employer failed to instruct the employees who were working in an approximately 10 foot deep by 10 foot long by 8 foot wide excavation without shoring, exposed reinforced steel, and unguarded machines  a way to avoid the unsafe work conditions.  Condition was observed on or about January 20, 2022.

29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:  a) Jobsite located at 1719 Seybert Street, Philadelphia, PA.- Employer did not ensure that a safe means of egress was provided within 25 feet of lateral travel in the working area, exposing employees to various hazards associated with not being able to safely egress from an excavation in case of an emergency.   Condition was observed on or about January 20, 2022.
