29 CFR 1910.178(l)(3): Powered industrial truck operators did not receive initial training in the following topics, specified in paragraphs (l)(3)(i)(A)-(l)(3)(ii)(A) of this section: Location: 6801 West Side Ave. North Bergen, NJ 07047 - Shipping Warehouse a) Employees were exposed to struck by hazards while conducting forklift operations without having a complete understanding of the operating instructions and precautions listed in the operator's manual to include specific training on surface conditions such as ramps and other sloped surfaces that could affect the vehicle's stability. On 01/15/2022 an employee was struck and injured by a forklift after it rolled down a ramp. Condition noted on or about 01/19/2022.
