29 CFR  1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  a) Line 1 Bread slicer: On or about January 16, 2022, an employee was exposed to cuts or lacerations while performing minor adjustments and clearing a jam on the bread slicer.  The employee did not utilize cut resistant gloves while reaching into the slicer.    b) Pan Conveyor Catwalk: On or about January 19,2022, an employee was exposed to crushing and pinching injuries while lowering a folding platform attached to a catwalk.  The employee was not provided with impact resistant gloves while lowering the platform.

29 CFR  1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures;  a) Bread Lines 1-4: On or about January 16, 2022 and times prior, employees lock out the bread lines to clean the machines.  The employer recently changed to new bread slicing machines and did not provided retraining to employees on the updated LOTO procedures.
